The buzzing/hissing is present in backgrounds noise, and in the tones and lower volume levels.
Bad Preamp tube or am I need of a noise gate and some form of volume control to turn up the amp more?
The fizzy tone pretty lessens at higher volumes, but the background noise is still there.
What is the most likely culprit?

The hissing is most likely preamp hiss which is typical of most high gain amps. Some more so than others. That being said... my '09 QR is quieter than every other high gain amp I've played. I do use a Decimator unit to eliminate the preamp hiss and any pedal, cable, ground or other source of buzz or other noise. The buzz you're hearing could be a preamp tube, most likely the primary gain stage. You can buy tubes (I recommend JJ's) specifically designed for this gain stage of high gain amps. They're quieter and offer a touch more gain than standard 12AX7's. The issue could also be a bad or cheap cord, or a noisy pedal (bad power supply). You're not playing a guitar w/single coil pup's are you? They can be noisy and vulnerable to picking up outside interference which is a lot more noticeable in a high gain amp.

Tone wise, remember to always have your Solo control at 9 o'clock minimum. Even though you're not in Solo mode, this control interacts with the Master. At low volumes, the amp will sound less fizzy than you're hearing just now, thicker, more like it should, as long as that control is up.
Hiss wise, I find my Competition extremely quiet, so unless you're using some nasty boost pedal or poorly screened cables, then yeah, it sounds like you might need some new tubes.
